DESCRIPTION: The Continuing Care Centre consists of 63 beds, focusing on Rehabilitation, Palliative, Complex and Transitional Care. We provide patient-centred care for patients requiring short term or slow stream rehabilitation, pain and symptom management, complex care and patients waiting for an alternative level of care. Currently, this position will be placed in Unit B (Transitional Care), however with organizational changes, may be integrated into all units of Continuing Care Centre. HOURS: Must be available for 11.25 and 7.5 hour tours, and other hours as assigned by the hospital, 24 hour rotation, 7 days a week SKILLS & QUALIFICATIONS REQUIRED: Registered Practical Nurse currently registered with the College of Nurses of Ontario Minimum on one year long term care and/or complex care experience within the last three years preferred Certified medication administration; this includes an approved medication administration course within the last five years, and successful completion of the medication administration orientation process Proven assessment, planning, implementation and evaluation skills related to complex care nursing Proven ability to work independently, utlizing effective organizational skills in a changing environment Demonstrated patient focused approach to care delivery Excellent Attendance Record Basic computer skills (Cerner, Groupwise) Our Vision is to Be the Best Community Hospital in Ontario Our 4 point Strategy to Be the Best Community Hospital in Ontario B ecome the Employer of Choice E ngage our Community S trengthen Current and Forge New Partnerships T arget a Bold Fiscal Strategy We invite you to call Human Resources, extension 2188, if you wish to confidentially discuss this vacancy in more detail and/or need assistance with your application.
